Results of the echocardiogram will be faxed to your office within two business days
CHECKLIST:
Call 217-757-6890 to schedule having demographic information available.
Prepare order with diagnosis.
Alert PCC scheduler of any special needs for the patient, i.e., mobility issues, need for oxygen, need for interpreter, infectious disease, etc.
Obtain necessary pre-certification according to patient’s insurance requirements;
If you wish Prairie to pre-certify, fax to 217-525-2535 Attn: Central Scheduling Dept.:
*Copies of insurance cards
*Copy of patient demographics
*Copies of medical information
Using the Instructions to the Patient form, record date/time/location of testing and give to patient.
Print map and give to patient.
Fax order and pre-certification information to 217-525-2535 to the attention of Centralized Scheduling.
